Your audio
What on-device means for a patient note
The audio stays on your computer. The text goes as far as you paste it.
The audio becomes text on your computer and is not uploaded. Around it:
The clipboard.
The text passes through it on its way to the field; the section for your system says what happens there.
The Dictate history.
The Dictate tab keeps a searchable history on your computer.
Where you paste.
Your records system has its own rules.
Lesskeys makes no claim about the privacy rules that apply to your practice; you, or your practice, are responsible for them.
Not a charting tool
Is it SOAP notes software?
Not a charting program. It turns speech into text wherever the cursor is; the structure is yours.
- No templates
- No patient list
- No records-system link
The same steps work for DAP or BIRP notes.
Check it
Medical words, and recorded visits
A general speech model, not tuned for medical terms.
It can get a drug name, a dose or an abbreviation wrong, so read every note. A text replacement fixes a term that keeps coming out wrong. For a recorded visit, add the audio file; no bot joins anything.
Tell the people you record; some places require everyone's consent.
On a Mac
Where the text waits on a Mac
On the App Store version the text waits on the clipboard until you copy something else.
The website download can paste for you and puts your clipboard back. With Handoff on, Universal Clipboard can paste it on a device signed in to the same Apple Account.

On Windows
Where the text waits on Windows
Lesskeys pastes the text for you with Ctrl+V and puts your clipboard back.
If the window refuses the paste, for example one running as Administrator, the text stays on the clipboard for you to paste yourself. The microphone is the only permission Lesskeys asks for.
Questions
Questions about dictating SOAP notes
Can I use it with my EHR?
Not directly. Lesskeys turns your speech into text and you paste it into the field, so it works with any system that accepts pasted text; it has no connection to any records system.
Who is responsible for the privacy rules when I dictate patient notes?
You or your practice. Lesskeys turns the audio into text on your computer and does not upload it, but it makes no claim about the rules that apply to you, and it cannot control the clipboard, your records system or where else the text goes. Ask whoever advises your practice.
Does it know medical vocabulary?
It is a general-purpose Whisper speech model, not a certified medical-vocabulary product. It can get medical terms, drug names and abbreviations wrong, so read every note before it enters a record.
Does dictating a SOAP note work offline?
Yes. A small starter model is inside the app, so dictation works from the first launch, and larger models download once. After that nothing about the dictation needs a network.
